Depending on the used skin you have to set the settings-level to standard or higher, in summary:
enable debug logging at settings->system->logging
reboot the OSMC device
reproduce the issue
upload the log set either using the Log Uploader method within the My OSMC menu in the GUI or the ssh method invoking command grab-logs -A

Ok, I’ll have a look at the cec settings, but why did this two issues come back after the latest update and why are they not always working in the same way?
Yes I’m sure that it was reintroduced by the last updates and the TV hasn’t got any updates.
I’m using the Vero daily so I’m sure that I would have seen this…

In a nutshell CEC uses a single line in hdmi cables to talk to each other. Part of this communication involves specifying what device is responsible for outputting the audio. When you get a volume bar that is different than what is expected this would likely be due to “something” happening that is causing a renegotiation of what device is suppose to be outputting and controlling volume, or another device going rogue and responding to volume control events not intended for it. This “something” could be a flakey hdmi connection, perhaps on the hotplug or CEC pins, or a device that has gone a bit sideways. A good place to start would be to unplug all devices from the outlet for five minutes and then power them back up. This will reset the CEC bus. If this does not improve the situation the next step would be to replace cables and maybe clean contacts with an electronics contact cleaner (preferably) or 90% isopropyl alcohol (remove power first and leave unplugged until dry). If the problem still persists after that then you are down to looking for recent firmware updates that may have bugs on all devices in the chain and maybe removing devices to see if the problem goes away when one of the devices is no longer plugged in.
